Description
HeadSlate is a fully offline headache fact diary for recording what you actually observe during headache and migraine episodes. It does not diagnose, predict, identify triggers, or recommend treatment. It simply organizes the facts you enter into a clear, reviewable personal history.
Start a current episode with one tap at a default intensity of 5, then add details when ready
End an active episode or manually add a past episode
Record start and end times, intensity from 1–10, and one or more pain locations
Log nausea, light sensitivity, sound sensitivity, pressure, throbbing, and visual changes
Record co-occurring factors such as poor sleep, a missed meal, screen time, a stressful day, a menstrual period, or travel
Add actions taken and optional notes without treating co-occurring factors as causes
Filter history by 30 or 90 days, status, and intensity, and search notes and recorded facts
Review episode count, total recorded time, average intensity, longest episode, weekly counts, intensity distribution, frequent symptoms, and co-occurring factors
See descriptive facts only, with no medical conclusions or predictions
Export a readable text summary, standards-compatible CSV, or complete validated JSON backup
Validate a backup before import, preserve existing records on failure, and recover from local backups
Clear all entries and local backups only after two confirmations
Use adaptive iPhone and iPad layouts with dark mode, Dynamic Type, RTL, and accessibility support
HeadSlate requires no account and includes no subscription, advertising, analytics, tracking, cloud sync, server, or network client. It requests no sensitive permissions such as HealthKit, location, notifications, or Photos. Episodes, symptoms, co-occurring factors, and notes stay on your device. A file leaves the app only when you explicitly use a system share or file interface.
