Get your care on the go with the Horizon NJ MyCare app. The Horizon NJ MyCare app is intended for use by Horizon NJ Health and Horizon NJ TotalCare (HMO D-SNP) members in New Jersey.
With the free Horizon NJ MyCare app, Horizon NJ Health and Horizon NJ TotalCare (HMO D-SNP) members can:
View covered benefits, including medical, pharmacy, behavioral health, dental, vision and more
View, download or request a new member ID card
Change your PCP
Find a doctor, dentist or other health care provider
View your Care Manager, if you have one
Find community support and resources available to you
Learn about additional services available to Horizon NJ TotalCare (HMO D-SNP) members only such as:
