Kaci, Chivas and Lara
Form Health is great. So thankful I was referred to them. I am so thankful for my Medical team.. Kaci, Chivas and Lara. They are so caring, understanding, supportive, and always there for me. I mess up regularly, but they are right there to get me back on track.. again and again. If you haven’t tried Form Health, you’re missing out.
Unethical and potentially illegal
Forced by my job and insurance to use this if I want access to medication already prescribed to me by my in-network doctors with my in-network pharmacy. When (not if) there is a class action lawsuit I will absolutely be joining it.
Services are great, app is not ideal
I really enjoy the services offered through the nutritionist and the doctor. The app is good if you're uploading meal pictures otherwise it's hard to use. You can't see individual weight logged unless you're online on web. It's hard to upload files and labs too.
It's a trap
Form health is a trap. Their business model is minimizing GLP-1 medications spendings for your employer. Your health is not a priority at all. They just pretend they are helping by intrusion into your private life. Form Health is holding you hostage just because your employer decided to try saving money, not even because you made this decision. And moreover, I expect Form Health does not even actually saving money for the employer, because Form services do cost money.
No Sync with Apple Health
FORM as a service is fine. My insurance requires it for a prescription, and the humans have been great. My complaint is they send you a scale to make sure your weight is accurate, and the scale syncs directly to FORM, but there’s no way to sync back to Apple Health. I’m okay with them insisting on using their scale but it’s terribly annoying that I can’t easily get my weight data into Apple. This is a straightforward integration missing from the FORM app.
App works just fine, program is great
I have gotten wonderful support from my team and have had no issues with the app. I was surprised to see the current Meh rating. I’ve worked with Form for 8 months now and plan to continue to do so as I get closer to my health objectives. I’m down 40 lbs, my broken foot has finally healed, plantar fasciitis is gone, my labs look great. I couldn’t be happier, for me this company is a great fit.
Technology Failure
Don’t understand how this company puts out this app with a straight face, need technology and customer mindedness people to join the company and have a voice. This app is unable to integrate with any other health or fitness apps. Poor functionality and worse design (a scroll wheel for dates- I kid you not). Amateurishly put together.
App is great, but share weight?
The app and program are great! But, you need to use a custom Withings scale that logs your weight into the app via cellular. That part is good, but if the app could offer exporting/integrating the weight into the Apple Health app, then all other apps could use the data like a normal Withings scale would. This would save needing to step on two scales just to log data outside of the Form Health app.
Needs work
As someone who has used multiple apps to track food,Weight etc this is lacking in many ways. To track food- an app should reflect what exactly and nutritional info and serving amounts one has consumed. On a positive note - love that I can use multiple devices to log info.
Basic
The app itself is ok. The camera is faulty in the app so I end up taking the pictures of my food then uploading in the app. Sometimes the camera in the app will fail to upload the image so you have to upload a picture from your camera roll. The one common sense feature I would think the app could do is allow you to go back and add pictures when you didn’t get to put in a food log entry. Since the camera in the app is unreliable, I just take the pictures of my food and log the food entries later, but life happens- you get busy or forget. If I forgot to upload my Monday food log entry then I should be able to add it in..

Annoying app
No appointment history, no way to msg the medical team, no way to enter weight for previous dates - really the worst thing ever, not to mention their service
Program is fine, major app flaw
Why on earth does the app not send notifications when we receive messages?? It’s the primary form of contact between me and my health team. Yes, app notifications and badges are turned on and I’ve restarted the phone. Quite frustrating to miss messages for days on end while I’m waiting for a response!
App is good, program is okay
I joined this app out of recommendation. I was hesitant to try. I've been on these apps that are supposed to encourage you to eat healthily and exercise. I've been on this app since December 2023 and I slowly lost weight but still in a rut. I recently canceled my program because it was starting to become less encouraging. I may go back but I think taking control of my own path for now is the best.
Effective and helpful
The app and the staff have both very helpful for weight loss. The app is simple but has the necessary components to track weight, food and share resources. The staff are always helpful, easy to reach etc. Usually I don’t prefer telehealth, but for weight loss I recommend this over traditional doctors because they take a more holistic approach and you don’t have long wait times, delays etc.
Worst Health App
This has probably been the WORST HEALTH APP I have ever experienced. Absolutely zero communication absolutely zero notifications of any kind. Compared to other health apps out there right now who are immediately responding to you same day - who are on top of everything - who are consistently updating you on every single step - form health app has some serious upgrading to do. I asked for my $149 refund and I still have not received a single response on their chat and via email. A whole week and a half has gone by and still nothing. I have not even gotten to the point of an appointment. I would not recommend this app to anyone. Go with RO or with Hers - who actually take your money and make it worth while. FORM HEALTH GETS NEGATIVE STARTS!
App it terrible!
I’m trying to schedule an appointment and when I choose a day and time the screen turns dark and it has to be forced closed and reopened. There’s no way to get tech support and offloading and reloading the app doesn’t work.
C’Mon… do better.
The program has been good so far and I have nothing but positive things to say about it. I was forced to use this program because it’s the only way insurance will cover it. But overall, it’s been very easy and convenient. There’s nothing very impressive about the app. It is very basic. One issue I have in particular is in the messaging. When I message back-and-forth with my physician, it creates multiple duplicates of my message, and sometimes they’re out of order. Considering this is dealing with peoples medical records, I would hope they would have a more stable chat system.
Fantastic Program
I found this program through the Eli Lilly website and have been extremely satisfied with my experience. I love having a team of professionals who are committed to my goals. If I were to change one thing, it would be to add a macro tracker to the app. I currently use MFP but it would be cool to have it all in one place. Overall, I give them an A+.

I cannot schedule appointments several days after registration. called CSR, not fixed.
App needs some upgrades
On many other weight loss apps, you have the ability to add what you eat by day, instead of adding pictures Willy Billy with no dates or meals to show what they belong to. Also, adding the ability to loss your water intake would be very beneficial. If you’re trying yo emulate my fitness pal or Noom, you have a ways to go.

My nausea was dismissed and the dr and NP didnt care
I was experiencing nausea and I wanted the doctor and nurse practitioner to care about my symptoms and what I was experiencing with the nausea. The nausea was all the time and didn’t seem to go away.
Great Program!
FORM has transformed my life. You have the support of board certified medical professionals. I have lost 30 pounds in 6 months. The app is easy to maneuver and a great way to find recipes, and text your doctor and RD while on the go. It also has been a sustainable program and you get out of it what you put in to it. The support and accountability has been a game changer for me!
Lack of a 1-800 number: very long onboarding
Compared to Ro, not very proactive in the onboarding process. Call One Medical to transfer primary care files and it would take 15-30 days. On customer support, there is a self-checkout for medical records. When attempting to upload to the Form Health link through numerous back and forth chats over two weeks, discovered that you cannot upload files larger than 10MB and zip files are not allowed. Form Health doesn’t feel like it’s ready for prime time. It’s a shame that my employer has selected Form to be its sole GLP-1 management service. The lack of being able to talk to a real human is disconcerting for healthcare.
Recent Upgrades are Huge Improvment
Adding an updated review as the app has made some major updates. It is now much faster and it seems like the developers are listening to the users. The updated food journal experience is very much appreciated. Please keep up the improvements!
Buggy app
On e they get you in, app stops working. No way to schedule appointments. Won’t prescribe auto refills. Would abandon if employer insurance allowed.
So far it’s great
Wow! I’m shocked by these bad reviews. My insurance also required me to join to get any of the WL shots covered anymore (even though they had been covered by my Dr’s prescription). But after I got set up, I got an appointment pretty quick with the Dr assigned to me. It took about 45 minutes total for the initial apt. My dietician apt is this Friday, which didn’t take long either. My dr has already put in a prescription for me. True, you take pics of your meals, but I also enter what I’m eating in MFP (I do wish this ap had similar). I just screenshot that & upload. There’s a place for notes to put the date. So far 5 stars. ⭐️⭐️⭐️⭐️⭐️
Awful App
The worst app possible. Can’t even upload anything
Recent updates make rescheduling impossible
What ever they did on the most recent update the rescheduling is impossible to do. The screen won’t pop up and stays stuck at the bottom of the screen with no way to bring it up. Makes me have to make several attempts to reschedule because of how long support services takes to respond to reschedule requests.
